Waiver and Release of Liability and Agreement to Participate
This agreement and release form must be signed by all participants and their parents if the participant is minor, who wishes to participate in Phoenix Shotokan Karate Dojo practice sessions or sanctioned events.
In consideration of being allowed to participate in any way in the training programs, tournaments and
related events and activities, including transportation to and from such events, of the Phoenix Shotokan
Karate Dojo, I hereby: Agree that prior to participating, I will inspect mats, equipment, facilities and any competition pools or pairings to be used, and if I believe anything to be unsafe or beyond my capability, I will immediately advise my sensei or supervisor of such conditions and refuse to participate. Acknowledge and understand that karate is a martial art and that I might engage in activities that could result in serious injury, including permanent disability or death and a severe social and economic loss due to not only my actions, inactions, or negligence, but the action, inaction and negligence of others, the rules of the art of karate or conditions of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to me or not foreseeable at this time. Assume all risks involved in the art of karate and accept personal responsibility for the damages following such injury, permanent disability or death. Enter karate training or competition entirely of my own free will, and understand the importance of following the rules of the art of karate and all directions given me by my sensei, assistant instructors and
tournament officials. Certify that I am in good physical condition, and have no disease, injury, or other condition that would impair my performance or physical and mental well being in intense physical training or competition. Grant permission in case of injury to have a doctor, nurse or athletic trainer or other emergency personnel provide me with medical assistance or treatment for such injury. Release, waive, discharge and covenant not to sue the Phoenix Shotokan Karate Dojo, Shawn Sample, Maria Morales, assistant instructors or other participating karate organizations, their affiliated clubs and schools, their respective administrators, directors, agents, instructors and other employees, or volunteers of the organization, event officials, medical personnel, other participants, their parents, guardians, supervisors, coaches, sponsoring agents, sponsors, advertisers and if applicable, owners and leasers of premises used to conduct class or event, all of which are hereinafter referred to as “releases”, from any and all claims, demands, losses, or damages on account of injury, including permanent disability and death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
I/We, the parents or guardians of this minor participant have instructed or will instruct the minor participant to the above warning and conditions and their ramifications, and I/We additionally confirm and agree to all of the above statements, conditions, waivers, and releases and consent to this minor’s participation.
I/We have read the above waiver, release and understand that I/we give up substantial rights by signing it, and knowing this, sign it voluntarily.
